Tuition rates are determined according to the following registration information.
Discounted (by August 15, 2003): $150.00
Regular (after August 15, 2003): $175.00
On-site: $200.00

Group registration: multiple physicians from the same institute or practice receive a $50 discount (each) off the published registration fees, provided all registration forms and payment are received on the same date.

This tuition covers all scientific sessions, course materials, planned meal functions, and a certificate of credit. If your registration must be cancelled and your request is received in writing on or before September 5, 2003, your paid tuition, less a $50 administrative fee, will be refunded. No refunds will be granted after September 5th, 2003.
